The sheet of paper contains what looks like a handwritten journal entry.

In my experiments I have inadvertantly created something which should never
exist in this world. The artifact I have enchanted is capable of opening a
gate directly to the lowest reaches of the underworld. I lack the means to
destroy the artifact in the limited time I have left in this world, and so I
have hidden it away and surrounded it with traps as well as I can.
