There is an intricate mechanism of springs and gears inside.

This round metal locket is about three inches across. It is attached to a
thick brass chain. It is composed of two halves, joined by a clamshell
hinge. A sun symbol is engraved on the face of one half, and half moon
symbol on the other.

When a small catch is depressed, the locket opens, revealing a marvellous
spring-driven mechanism. A flat white face has the numbers one through
twelve printed around the edge. In the center of the circular face is a
post, on which a single arm is affixed. The arm slowly moves between the
number from hour to hour, indicating the approximate time. The mechanism
appears to be self-winding, whether through clever artifice or magical
contrivince it is difficult to discern. It emits a slow, regular "ticking"
sound.
